Have SIP trunks shared between 2 doctors. How is outbound caller ID set up in 7100 for each doctors extensions to send out their respective phone number?

Just go into the SIP Trunk and configure ANI Substitution.
Here is an example:
(config)#voice trunk T01
(config-T01)#match ani 8101 substitute 256-555-81001
