In the end, the provider did something on their end that fixed this issue, as it started working one day without us having made any real changes.

We have a customer who is experiencing a similar issue on a Adtran 908e router where VoIP calls are consistently dropping after 15 minutes on Polycom phones. The customer is using a Extreme switch and we are attempting to isolate the problem to determine if the issue is on our end or theirs.
