Call fowarding rings and then disconnects

I just upgraded an old PBX from using an old-school PRI to using a TA908e to take the local PBX's PRI, convert to SIP, and connect to a sip service.  

I can make and receive calls but when I call an extension that is called forward to an external number the external phone will ring, but when it's answered the call is disconnected.  

the only change was switching out the T1/PRI so I'm stuck looking at the TA908e for why it's causing this error.  

Can anyone point me in a new direction?
